A Journey Rooted in Care and Community

Horizon Family Medicine was born out of a deep passion for providing accessible, high-quality healthcare to our community in East Hartford. Founded by a doctorally prepared family nurse practitioner with over 25 years of experience, the clinic was established to bridge the gap in primary care services and create a welcoming space for patients of all backgrounds. Our motivation stems from a belief that everyone deserves compassionate, equitable care tailored to their individual needs, regardless of race, gender, or socioeconomic status.

This commitment to care is reflected in every aspect of our practice, from our inclusive approach to the strong relationships we build with patients. As a Black and female-owned clinic, we are proud to serve as a trusted healthcare provider and advocate for our community’s well-being.
